Don't you mean "the points are (-6,-3) and (6,-7)?"

Use the slope formula to find the slope of this line:
          -7 - (-3)
m = --------------- = -4/12 = -1/3
          6-(-6)
Now substitute -1/3 for m in y-k = m(x-h), and -6 for h and -3 for k:

y - k = m (x - h)
